Consent Form for Hypnotherapy Services

I acknowledge that Hypnotherapy is a gentle, self-improvement technique designed to promote relaxation and reduce stress. I understand that hypnotherapists do not diagnose medical or psychological conditions, prescribe medications, perform medical treatments, or interfere with care provided by licensed medical professionals.

I recognize that Hypnotherapy is not a substitute for medical or psychological treatment, and I am advised to seek care from a licensed physician or healthcare professional for any physical or psychological concerns. However, I understand that Hypnotherapy may complement ongoing medical or psychological care.

I acknowledge that the body has an innate ability to heal itself and that achieving deep relaxation can be beneficial to this process. I also understand that long-term imbalances may require multiple sessions to reach the relaxation level necessary for healing.

Agreement & Acknowledgment

By signing this form, I agree to the following:

1. Voluntary Participation: I am voluntarily participating in Hypnotherapy because I choose to do so.
2. Transformation Process: I understand that transformation is a process and may take time.
3. Personal Responsibility: My progress depends on how I care for myself physically, mentally, emotionally, and spiritually.
4. No Guarantees: No warranties or guarantees have been made regarding specific outcomes.
5. Refund & Expiration Policy: All purchased packages are non-refundable and must be used within 60 days to avoid expiration.
6. Accountability: I acknowledge that I am responsible for my own actions, decisions, and outcomes, and I release any blame toward myself or others.
7. Active Participation: I understand that I am not a "patient" but an active participant in my hypnosis experience.
8. Scope of Practice: I acknowledge that Hypnotherapist is not a medical doctor, psychologist, or mental health professional. Hypnotherapy services are non-therapeutic and are not intended to replace professional medical or psychological care.

Telehealth Services Agreement

I understand that telehealth Hypnotherapy services (remote or online hypnosis sessions) are offered as an alternative to in-person sessions. I acknowledge the following:

• Telehealth sessions use secure video conferencing technology to provide services remotely.
• The effectiveness of Hypnotherapy via telehealth may vary to individual experiences.
• I am responsible for ensuring a quiet, private space during my telehealth session.
• I understand that technical issues, such as internet connectivity problems, may impact my session, and I agree to reschedule if necessary.
• I acknowledge that telehealth services do not replace in-person medical, psychiatric, or emergency care.
• I understand that all telehealth sessions must be prepaid and are non-refundable once scheduled.

Membership Agreement (if applicable)

I understand that by enrolling in a Hypnotherapy Membership Plan, I agree to the following terms:

• Membership fees are non-refundable and automatically billed on a recurring basis.
• Membership benefits may include discounted sessions, exclusive content, and access to specific Hypnotherapy programs.
• Unused sessions do not roll over beyond the membership period unless explicitly stated in the membership terms.
• You can cancel your subscription anytime directly from your profile—no questions asked, no hassle, no obligation, no commitment. You are in full control. If you decide to return, reactivating is as easy as one click from your profile.
• Membership cannot be transferred to another individual.
• The provider reserves the right to update membership terms and fees with advance notice.

Cancellation & Rescheduling Policy

I agree to provide at least 48 hours' notice if I need to cancel or reschedule an appointment. If I fail to do so, I understand that I will be responsible for the full session fee.
